Mental Health Clinical Case Management

...Under the Mental Health Services department we have a Clinical Case Management program.

Through the initial intake process or throughout counselling an individual may be identified as benefiting from case management support services.

The Clinical Case Manager will meet with individuals to identify needs and can work to support referrals for other resources and navigation of various health care needs.

Services Provided:

support with both internal and external referrals
advocacy and support with navigating health care needs
applications to housing, residential care, treatment, employment, etc.
support with legal matters and court processes
